2. Personal details
First name, last name and phone number.
3. Company
Company name, address and USDOT number.
4. Billing
Billing address, card and acceptance of terms — entered by the customer.

Rules for assistants

Act only with the customer's permission and use their real details.
Never invent a USDOT number, address or phone number.
Hand over at billing: the customer enters their own card and accepts the terms.
